Health Instruction Form

New York State Law requires that all students new to McQuaid and students in 7th and 10th grade submit an up-to-date physical exam at the start of school. Further, all new students, including foreign exchange students, must demonstrate proof of full immunization prior to the start of school. The following immunizations are required:

DPT (Diphtheria-Pertussis-Tetanus) 3 doses

Tdap (Tetanus-Diphtheria-Acellular Pertussis) 1 booster (upon entering grade 6)

Polio (OPV or IPV) 3 – 5 doses

MMR (Measles-Mumps-Rubella) 2 dosesMeasles, Mumps, Rubella

Hepatitis B 3 doses

Varicella (Chickenpox) 2 doses

Meningococcal Vaccine 1 – 2 doses (1 for 7th grade, 2 for 12th grade)

Proper documentation of immunizations are closely monitored, as this is an important public health issue and concerns the well-being of all of our students and staff. It is imperative to comply with all immunizations that are required by law.

Further, New York State has revised some immunization regulations (effective 2015), and we will be evaluating all students for compliance with the new regulations. If your son is found to have a discrepancy, we ask that you respond in a timely manner to any additional medical requests.

Throughout the school year, the Nurses conduct vision, hearing and scoliosis screening on students if it was not done at their physician offices. Notice will be sent home if further evaluation is warranted.

If your son becomes ill or injured during the school day, we will notify you in the event that he needs to go home or follow up is indicated. It is important to provide us with the proper contact information and telephone number. You need JavaScript enabled to view it.. We are interested in your son’s total well-being – physical, social, and emotional. 

If your son requires any medication during school hours (prescription and/or non-prescription), a doctor’s order must be sent to the Nurse’s Office. (effective 2015, medical permission is required for over the counter medications.)  Prescription medications must be provided in a properly labeled container. We stock Ibuprofen and Acetaminophen for student use, but do require a note from your son’s doctor to dispense these medications.

Completed Interval Health History forms (Sports Triplicate) and a current Physical Exam (within the last calendar year) must be turned in at the Nurses Office prior to sports tryouts.*   As a reminder, there are no physical exams offered at McQuaid.

The completed Interval Health History must be signed by both parent and student.  The form may be mailed, faxed, emailed, or hand delivered to the Nurses Office, but it must be submitted at least one week PRIOR to the tryout.  A new Interval Health History has to be completed every time a sport is done.

The Nurse will also be in the office for the first day of EACH tryout, approximately one half hour before that tryout begins. Please do not wait until the last minute for clearance, as your son may not be eligible for tryouts if the necessary paperwork is not in order.  

*If you do not wish to come into the school for clearance, you may fill out the triplicate form and submit it by mail along with an up-to-date physical form (within the last year).  If the athlete has any outstanding injuries, a clearance note from the doctor should accompany these forms.  If all those requirements are met - the athlete will be cleared.

Ignatian Formation Meeting Dates

This MANDATORY meeting is required once per academic year for all parents of McQ student-athletes.

Doctor's Notes

If your son has been seen by the doctor for any reason that requires him to be excused from PE or sports, please send a doctor’s note into the Nurse’s Office. We will copy the note and dispense to appropriate teachers and coaches.

If your son has had surgery or sustained an injury that prevents him from participating in PE or sports, please send in a follow up note from the physician when he is cleared to participate again.

Lastly, please send in a copy of any regular physical exams that have been done. Physical exams are required for students in grades 7 and 10, regardless of athletic participation.

The role of the Nurse is to ensure student health and safety throughout the day. We request that parents and guardians communicate any health concerns to the nurse. This includes physical issues as well as any emotional or psychological concerns that your son be experiencing. The Nurse is an important part of the team that supports students and encourages continuity in the educational setting.


We do stock some over the counter medications, such as Advil, Tylenol and Tums. We do NOT have decongestants or antihistamines. If your son requires an over the counter medication during school hours, please fill out the over the counter form, sign it, HAVE YOUR PHYSICIAN SIGN IT, and return it to the Nurses Office for us to keep on file. This form will have to be renewed annually.

If your son requires a medication other than our stock meds, please send the medication into school in the original package labeled with your son’s name. We will dispense the medication to your son as needed, with a doctor’s note.

Further, if your son is an asthmatic or has a history of asthma, please supply us with an inhaler (or let us know if your son is carrying one.)

Please send in an Epi-Pen if your child has a life threatening allergy. This also requires a physician note with detailed instructions for administration.

As a reminder, any controlled substances that are prescribed for your son must be brought into school by a parent or guardian with a note from the prescribing physician.

At the end of the school year, please pick up your son’s medications. Expired medications and medications left behind will be discarded.

Drug & Alcohol Policy

Alcohol and drug abuse are serious societal problems. McQuaid Jesuit believes in educating the whole person and
consequently feels obligated to educate the students to the dangers related to alcohol or drug dependency.
Any student involved in the sale of controlled substances is subject to immediate expulsion. Any student using
and/or possessing alcohol, marijuana or nonprescription drugs, or manifesting their effects at school or at any
school-related activity will be suspended from classes and/or sent home. The student's parents will be notified, and
he will be barred from classes until a parent-administrator conference has taken place. If this is the student's first
offense, he will normally be placed on disciplinary probation and serve ten days of Jug, and a professional
assessment of his involvement with chemical substances may be required. Arrangements to complete this
assessment must be made within two weeks of the incident. The reports and recommendation must be reported to
McQuaid Jesuit. The student will be bound to any recommended treatment as part of his probation. Repeated use
or possession of alcohol, marijuana, controlled substances, or non-prescription drugs makes a student liable to

The school may require students to submit to a Breathalyzer test if recent use of alcohol is suspected. Failure to
submit to the test will invoke the above policy.

Student athletes who violate the alcohol/drug policy in season are also subject to the athletic policy on drugs
and alcohol. See Below.

Interscholastic Athletic Policy on Drugs/Alcohol

  • If a student athlete on an interscholastic athletic team is found by a teacher, administrator, coach, or
    officer of the law to have been in possession of or under the influence of alcohol, mood-altering drugs, or steroids
    during the season (the season is defined as the first required team practice through the last team practice, game or
    event), the following sanctions will be invoked:
  • First Offense: The student athlete’s name will be reported to the Athletic Director and the Dean of
    Students and a letter will be sent home notifying the parents that the student athlete will be suspended from
    interscholastic competition for a period of no less than 25% of the total season’s competition*, immediately
    subsequent to the offence. The student athlete must submit to a professional chemical assessment by an
    accredited agency approved by the school. Release of the results to the school is required.
  • Second Offense: The student athlete’s name will be reported to the Athletic Director and the Dean of
    Students and a letter will be sent home notifying the parents that the student athlete will be prohibited from
    participation in interscholastic athletics for one calendar year from the date of the offense.
  • The above athletic sanctions do not supercede or replace penalties that may be imposed via the Student
    Conduct Code or additional sanctions from an individual coach. McQuaid Jesuit regards involvement with drugs or
    alcohol to be a serious violation that normally results in penalties including suspension, probation or even
  • Any student athlete wishing to appeal an athletic disciplinary action must submit a written request, to
    the Principal, within one week of the notification of disciplinary action. This request must include the reason(s) why
    the infraction should not be disciplined as outlined.
  • Any referral initiated by a family member of the student athlete himself will not lead to any athletic
    disciplinary action as long as the student athlete agrees to cooperate with an appropriate in-school assessment of
    his situation.

    *During the suspension period, a student athlete may attend athletic events, but may not wear team attire or
    stand/sit with the team. 

Appeal Process

Any student athlete wishing to appeal an Athletic Disciplinary action must submit in writing to the Principal of
McQuaid Jesuit, within two days of the notification of the disciplinary action, the reason(s) why his training
infraction(s) should not be disciplined as outlined.


Coaches are in charge of the ordinary discipline of their teams. The student athlete is expected to comply with
whatever regulations or disciplinary measures the coach may impose. Faculty, Staff members and Coaches should
always be addressed in a polite and respectful manner. Arguing with a teacher, staff member or a coach in the
presence of other students is at variance with the conduct expected of a McQuaid Jesuit student. Open defiance,
disrespect or insubordination to any teacher, coach or staff member could result in suspension or expulsion. Before
any such problem might arise, the student is to maintain courtesy toward the teacher. At the end of the athletic
activity, if need be, he may consult with the Director of Athletics for mediation of the problem.

McQuaid Jesuit reserves the right to discipline students who fail to observe established rules and regulation while
traveling as a team on school or charter buses. Students should always behave in a manner consistent with the
environment of McQuaid Jesuit while riding the school buses.

The following is a partial list of infractions that make a student liable to be expelled:

  • Stealing anything or "holding" property of another student.
  • Possessing an unauthorized school key.
  • Destruction of school or personal property.
  • Disruption of school activities, e.g., false alarms, smoke bombs, stink bombs.
  • Doing anything that endangers the good name or reputation of McQuaid Jesuit, whether on or off the
    school premises.
  • Endangering the wellbeing of any person (e.g., bringing any weapon to school, setting off firecrackers).
  • Repeated use or possession of alcohol, marijuana, controlled substances or nonprescription drugs.
  • The sale of controlled substances.
  • Open disrespect, defiance or insubordination to any teacher or staff member.

2017 Fall Sports Tryouts

Opening day for J.V. and Varsity sports tryouts is August 14, 2017.
Opening day for Freshman Football tryouts (should a team be formed) is August 21, 2017.
Opening day for Modified Football tryouts is August 23, 2017.
Opening day for all other Modified Sports is September 5, 2017.

Students DO NOT need to come to school to be cleared for sports participation.
Sports forms may be faxed, emailed, mailed or hand delivered, but they MUST
be sent in with a current physical exam.
Please take care to fill out the form thoroughly and sign in all designated spots.
