NYS Requirements

Physicals

The state of New York requires physicals for all new students and all 10th grade students.

Immunizations

All new entrants, including foreign exchange students, must have the required immunizations before they will be permitted to attend McQuaid Jesuit. Immunization is required for the following:

1) DPT (Diphtheria-Pertussis-Tetanus) 3 doses

2) Tdap (Tetanus-Diphtheria-Acellular Pertussis) 1 booster (at 11years of age)

3) Polio (OPV or IPV) 3 doses

4) MMR (Measles-Mumps-Rubella) 2 doses Measles containing vaccine

5) Hepatitis B 3 doses

6) Varicella (Chickenpox) 1 doses of vaccine or confirmed case of Chickenpox

Athletes

All athletes trying out for fall sports must have all of the above information on file at McQuaid Jesuit by the first day of tryouts. The McQuaid Jesuit Athletic Form (Triplicate) must be on file no earlier than thirty days before the first day of tryouts. This form must be handed to the nurse, in person, by the student athlete on our scheduled summer clearance days.

Medications

If a student is to receive medication during school hours, the school nurse will store the medication and provide the service once she has the following:

Signed authorization from the physician telling the name of medication, the dosage and the duration of time involved

Signed statement from the parent asking that the medication be so given and waiving all claims that might arise because of the administration of these drugs.
