REQUIREMENTS FOR PARTICIPATION IN A SPORT

            Each time a student would like to tryout for a sport, he must complete a triplicate Athletic Form. The Athletic Form includes the Interscholastic Athletic Policy on Drugs and Alcohol, and must be signed by both student and parents.  The student may pick up these forms in the Nurse’s Office, or they may be downloaded from the link below.  If you choose to download this form, you MUST provide three copies, and submit all three to the Nurse’s Office.  One copy may be downloaded, completed, and copied twice for submission to the Nurses’s Office.

            To be eligible to participate in a sport, the student must have a recent (within the last calendar year) physical examination on file at McQuaid.  If the student has sustained any injury after the time of the physical exam, he must provide written clearance from his physician.  Tetanus vaccines must also be current (within the last 10 years.)

            Triplicate forms are to be turned in to the Nurse’s Office ONLY, and they are due a minimum of two weeks before the start of the sport tryout.  Students receive final clearance for participation from the McQuaid nurses.  All requirements for health clearance must be completed, not pending, BEFORE tryouts. A student may not tryout unless all necessary forms are in! 

If a student’s physical examination is out of date, and he is unable to obtain a physical from his own physician, he may have a physical examination done at school.  A Nurse Practitioner conducts physicals at McQuaid three times during the school year -- August, November, and February.  The student MUST schedule an appointment with the McQuaid nurse for these physicals several days prior to the examination, and provide written permission from parents.  Please note - August physicals are 1st come, 1st serve - no appointments are made. Multiple announcements are made at school regarding the date of these physicals.
