IMMUNIZATION REQUIREMENTS

 A complete immunization record must be provided in compliance with New York State Compulsory Immunization Law at the time a student enters McQuaid.  All students, with the exception of those exempted in accordance with the law, must be immunized against diphtheria, measles, mumps, rubella, and polio.  For students entering seventh grade after 9/1/00, a full series of Hepatitis B vaccines must be documented.  For admission to school, each student must have received:

*  3 or more doses of diphtheria toxoid

*  3 or more doses of trivalent oral polio virus (TOPV) or 4 or more doses of inactivated poliomyelitis (IPV)

*  2 doses of live measles virus vaccine -- the first dose administered after the age of 12 months, and a second dose administered more than 30 days after the first dose and after age 15 months

*  1 dose of live rubella virus vaccine administered after the age of 12 months

*  1 dose of live mumps virus vaccine administered after the age of 12 months

*  For students required to have Hepatitis B, either 3 doses of Recombivax HB, or Enerix B, or 2 doses of Adult Hepatitis B vaccine (Recombivax) for children 11 to 15 years of age.

 

In the event that an immunization series is incomplete, a statement from the physician is required explaining that the student is in the process of completing requirements, or is exempt for one reason or another.  A student may be exempt because the immunizations may be detrimental to the student’s health, or the parent/guardian may express an objection based on a sincerely held and practiced religious belief.  In the case of a religious exemption, a written request must be submitted and approved by an attorney. 

            No students will be allowed in school without a complete immunization record.  Only students new to the district from another state or country will be granted a 30-day grace period to comply with these requirements. 

            All entrants or re-entrants who have lived or traveled outside the United States, Canada, or Western Europe, will need proof of freedom from active infectious tuberculosis prior to entering school.  This may be a doctor’s report of a negative chest x-ray or a negative Mantoux skin test done within the last six months.

            Any student participating in sports must have a recent tetanus immunization (within the last ten years.)
